The OptimumRX Card program is offered to you with the
following Terms and Conditions. In these Terms and Conditions, “you and “your” mean
each person who has purchased an OptimumRX Card Medi Plan I or Medi Plan II.

your initial enrollment, OptimumRX Card will reimburse
your membership fee within thirty (30) days. If you cancel your membership after
the initial thirty (30) day period, there will be no refund of any fees you paid. |
your membership only for your own personal use and for
any covered household member. You and OptimumRX Card agree that there are no additional
third-party beneficiaries under the OptimumRX Card program. |

• Membership Term and Cancellation
You must pay the established membership fee when you sign up for the program in
order to enjoy the privileges of the OptimumRX Card.
You may cancel your membership at any time. If you wish to cancel your membership,
you must do so in writing. Mail cancellation letter and your OptimumRX Card to:
1704 Tower Drive, Edgewater, NJ 07020. Your membership will terminate effective
as of the date OptimumRX Card receives your notice of cancellation. If you cancel
your membership within thirty (30) days of

OptimumRX Card may cancel your membership if the OptimumRX Card program is discontinued
for any reason and OptimumRX Card would, in that event, provide you with at least
thirty (30) day’s written notice of the cancellation at the address you have provided
to us. However, if you fail to pay membership fees, as required, or if you violate
any of these Terms and Conditions, OptimumRX Card reserves the right to cancel your
membership immediately.

Your membership and benefits are not transferable or assignable without our express
written consent. You agree to use

• Confidentiality
OptimumRX Card ensures that customer prescription information remains strictly confidential.
OptimumRX Card works with agents and affiliated, also bound to strict confidentiality,
to perform certain functions under the program. AS an extra service to you, as a
part of your enrollment in this program, we may send e-mails, direct mailings, or
telephone you with offers or information that may be of interest to you. For these
purposes, we may use outside processing companies as OptimumRX Card agents but they
are also bound to strict confidentiality. Your privacy is of utmost importance to
us. |
